2025 Contract Renewal Information

There are two contracts we need to have you complete for the new year.

1. Counselor Contract - We need ONE of these per person.

2. Practice Contract - We need ONE of these per practice.

   1. If you are in individual private practice, you will sign both the counselor contract and the practice contract as yourself. If you operate under your name (versus a “practice name”), you will use your name in any “Practice Name” fields on the contract.

   2. If you are own a group practice and see GCN clients, you will sign both contracts (and your staff each only need to sign the Counselor Contract).

   3. If you own a group practice and are not personally seeing GCN clients, you will sign the Practice Contract and each of your team will sign the Counselor Contract individually.

   4. If you are in a group practice but do not own/run it, you will sign the Counselor Contract and then you will need to get the practice owner/operator to sign the Practice Contract. Feel free to send your practice owner the link to the appropriate Practice Contract. Keep in mind they only need to sign this once for your entire practice.
