- What led you to become a counselor?
- What theoretical orientation do you use?
- What population are you interested in working with and how did you become interested in that population?
- Any trainings or specialities that you have?
Copy of your Picture ID
Copy of your malpractice insurance
Copy of your current resume/CV/Vita
Combined copy of your intake paperwork, which should include: Informed consent, no surprises act, HIPAA statements, general information, etc.
Your mental health license info (state, number, license type, license number) - you will add this info to the form, but won’t need to send us a copy of the physical license

Once you have completed the application, you will have two follow-up steps:

An email invite to a background check will be sent to you within a few days of submitting your application. Please complete that so we can verify your information and advance your application.
Please send a reference request link to two of your professional connections. You can text or email the link, whatever is easiest for you!
